You are responsible to verify your insurance. All payment is due at the time of service.

• Call the customer service number on the back of your insurance card.

• Verify Tamara Wirth is an approved provider with your insurance company.

• Ask if you have a deductible, co-insurance or copay.

The deductible is the amount you pay for covered health care services before your insurance plan starts to pay. For example: If your deductible is $2000. You would pay the first $2000 of covered services (the full amount for the appointment) before your insurance plan starts to pay.

Co-insurance is a percentage of the amount the insurance company allows to be charged for services. You start paying coinsurance after you have paid your plan’s deductible.

Copay is your share of the costs of a health care service. This is usually a preset amount set by your insurance company.
